HOW TO APPLY:
1. Please make an appointment at (727) 683-1467
2. Bring the following required documents:
- Proof of Name Change: original marriage certificate, divorce certificate or a name change court decree.
- Recently issued US passport. Make sure that the your passport was issued less than one year ago. If it is more than one year since the date of issue, you must apply for the passport renewal or for the new passport (if applicable).
- Proof of identity: Valid Driver license or ID card
HOW TO PAY:
– Service fee can be paid with cash, check or money order. We also accept credit cards with 3% surcharge.
– Government fee MUST be paid with personal check or money order payable to US Department of State.
